Maintenance Task Risk Assessment Template

Purpose. Use this template to assess and control the risks of a non-routine maintenance task or a routine task performed under unusual conditions.
1.How to use this template
Complete a risk assessment before any non-routine task such as jacking on a sloping apron, engine runs at night, fuel tank entry, working at height on a stabiliser, or a task on a type new to the crew. The supervisor leads the assessment with the crew and files it with the work pack.
2.Identifying hazards
Walk through the task step by step and list the hazards to people, the aircraft and equipment, including fuel and fluids, stored energy, electrical, height, noise, manual handling, foreign object damage and human factors such as fatigue and time pressure.
3.Assessing risk
Rate each hazard for likelihood and consequence using the matrix in the safety management system to give a risk level. Risks rated high must be reduced before the task proceeds.
4.Controls
Choose controls following the hierarchy: eliminate, substitute, isolate, engineer, administer, then personal protective equipment. Record who is responsible for each control and confirm it is in place before starting.

- Administrative controls such as permits and briefings
- Personal protective equipment as the last layer
5.Approval and review
The supervisor approves the assessment and briefs the crew. If conditions change during the task the assessment must be reviewed before work continues.
