Deferred Defect Request Form

Purpose. Use this form to request and record the deferral of a defect that is permitted to remain open under the approved data or minimum equipment list.
1.When to use this form
Complete this form when a licensed engineer has assessed a defect as eligible for deferral and the operator has agreed. The completed form is attached to the entry in the Defect and Deferred Defect Log and the limitation is recorded in the aircraft's maintenance release.
2.Who completes it
The assessing licensed engineer completes the defect and justification sections. The operator's representative confirms agreement. Planning records the due limit and schedules rectification.
3.Justification
State the approved data or minimum equipment list reference that permits the deferral, the operational limitations that apply and any maintenance actions required before or during the deferral period.
4.Limits
Record the rectification limit in calendar days, flight hours or cycles as stated in the data. The limit must not be extended without a new assessment and a new form.
5.Close out
When the defect is rectified, the work order number and date must be recorded and the log entry closed.
